Combined Initial Disclosure Document - key facts about our serviceIt is important that you have information at an early stage including the nature and scope
of the services that we may offer. Please read our Combined Initial
Disclosure Document and print off a copy or store it on your hard drive
for future reference. 2. Obtain fund supermarket forms and documentationYou can find information on the fund supermarkets including the funds available following the links under 'general information' below. Where available, you can log into the fund supermarket website using the links below. Please refer to the other process steps below - even if you transact online you may still need to send paperwork to Clubfinance. To benefit from our discounts, you will need to ensure that Clubfinance is indicated as your intermediary (our reference for Fidelity FundsNetwork is 812935). For some fund supermarkets, all the forms and information necessary are available from the Clubfinance website (see 'Download from Clubfinance website' in the table below). Alternatively we can provide information and applicable forms via e-mail. Please follow the relevant link under 'e-mail request form' below.
If you are interested in any other fund supermarket, please e-mail us, and we will investigate for you. 3. Read the fund key features documentationIt is important to read the fund key features associated for your chosen fund(s) and either print off a copy or store it on your hard drive for future reference. For an increasing number of funds, this will include a Key Investor Information Document (KIID). If you feel you need any form of advice you must not use our service, we offer an execution-only service for individuals who require no advice. In addition, please contact us if you would like a copy of the relevant Simplified Prospectus(es) for your chosen fund(s) (where available). 4. Read the fund supermarket's terms of business and/or key featuresIt is important that you read the fund supermarket's terms of business and/or key features, and either print off a copy or store it on your hard drive for future reference. 5. Read our (Clubfinance Ltd) Terms of Business and any additional risk warningsIt is important you understand our Terms of Business. As we rebate high levels of commission, we provide a very specific service. To understand our service fully you must read our Terms of Business. You should either print off a copy or store it on your hard drive for future reference together with any additional risk warnings that are relevant to your investment. Our Terms of Business are applicable to all transactions with Clubfinance Ltd and can be found using the following link: There may be additional risk warnings applicable to your investment, such as for ISA transfers. These are available here: 6. Check the Commission Statement (and Personal Illustration)For each fund supermarket other than Fidelity FundsNetwork a generic Clubfinance Commission Statement is available as a pdf document on the Clubfinance webpage for that fund supermarket (see above). For Fidelity FundsNetwork, commission information should be shown within the Personal Illustration section of your personalised Key Features Document. This should reflect the 100% rebate of our initial commission to reduce the initial charge. If for any reason your online purchase statement shows initial commission being paid, please contact us immediately and we will rectify this. Our renewal commission before rebate will also be illustrated. We rebate 75% of our renewal commission annually by cheque. You should retain this information for future reference. If you have not received commission information for your investment, please use the link below 7. Print and complete the Clubfinance Client Declaration formIt is important that you fully understand our service and confirm to us that you have taken due care and diligence in choosing your investment. The Clubfinance Client Declaration provides an important checklist and evidences to us your acceptance of our Terms of Business. If we do not receive a signed copy of the Client Declaration form, we will be unable to process your transaction. You should also either print off a copy or store it on your hard drive for future reference. Each time you send us a paper application form or forms, you should include a Client Declaration. If you are using the Fidelity FundsNetwork website to invest on-line using a debit card or to switch between funds for example, you need only send us a Client Declaration the first time you do this. Our Client Declaration form can be found using the following link: 8. Proof of name and addressIf this is the first time you are using our service (or we have only arranged life insurance for you), you must ensure that you send Clubfinance relevant proof of your name and address. Due to strict money laundering regulations we cannot process your application without these. If you have changed your name or address since last using our service you must also supply further identification. Our identification requirements and information regarding what forms of identification are acceptable to us can be found using the following link: If however you do not have any of the forms of identification listed, then please send an e-mail explaining your circumstances to our e-mail address shown on our contacts page and we will make our best efforts to find a suitable alternative means of verification for you. 9.
